Position Title: Child
and Family Support Worker- SHAPE
Salary: $17.00-17.50/hour
depending on years of experience/education level.
Schedule: Full time/35 hours per week
PRIMARY FUNCTIONS:
The Yates Family Support Worker will fulfill the home
visiting function within the SHAPE program.
SPECIFIC DUTIES:
· Carries a caseload that
requires weekly visitation and crisis intervention in the client’s home
environment. This may include schools, homes, clinics and other related sites.
· Responsible for assuring
that the rules and recommendations of the Office of Children and Family
Services, and the Agency as they are applicable to direct services and ASFA
guidelines for children and families, are adequately and appropriately adhered
to.
· Responsible for the
completion of all Family Development Plans, and other supporting documents
within the accepted timeframe outlined by standards.
· Relates to community
agencies, coordinating services as needed to facilitate family rehabilitation, reunification,
and child permanency.
· Ensures compliance with
each family’s service plan as outlined on the FASP in Connections, NYS
Regulations, funder contracts and referral information through contacts with
family members.
· Provides regular
outreach family support and education sessions and suggests appropriate
resources for family members.
· Maintains electronic
reporting and program statistical information as required by the Agency and all
funding sources.
· When necessary,
supervises or observes visitation between family members to assure the safety
and best interest of all members.
· Participates in
in-service training to develop family casework skills.
· Provides after-hours
on-call coverages for the CCSI program (in program rotation).
· Is responsible for any
other duties necessary for the program, within their abilities, as assigned.
QUALIFICATIONS:
Bachelor’s degree in social work or related field. Prior
experience is preferred.
Some education/human services experience, either through
employment or educational internships including working with Mental Health or
Child Welfare. Prior experience providing crisis intervention services and
possess thorough knowledge of service delivery systems in Yates County.
ADDITIONAL REQUIREMENTS:
- Adheres to all applicable federal and state laws and
regulations including, but not limited to, those governing
confidentiality, privacy, program, billing, and documentation standards.
All duties must be performed in accordance with CCDR’s corporate
compliance and ethics program.
- Possess excellent verbal and written communication
skills.
- Ability to multi-task and prioritize duties.
- Willingness to foster agency, department and program
wide cooperation and teamwork through use of positive/constructive
communication techniques.
- Ability to maintain absolute confidentiality regarding
all records reviewed including consumer records, employee records and
billing records.
- Proficiency and experience with PC’s and Microsoft
applications.
- Ability to analyze and interpret data and to handle
problem resolution.
- Possession of a valid NYS Driver’s license and a
driver's record considered acceptable by agency and insurance carrier.
- Continuous use of a reliable, registered, and insured
vehicle.

literacy and other communication needs.
Ability
to meet the following physical requirements with or without reasonable
accommodation:
- Use hands to manipulate, handle, feel, and control items
or equipment, including motor vehicles.
- Stand, sit, walk, bend, stoop, kneel, and reach.
- Climb up or down stairs.
- Able to reach above or below shoulders.
- Occasionally lift or move objects weighing up to 30 lbs.
- Sitting at a desk or in a vehicle for long periods of
time to perform certain job functions.
- Be able to read, write and interpret written documents.
