Senior Housing Supervisor

PRIMARY FUNCTIONS

Under the supervision of the Director of Residential Services, the Senior Housing Supervisor’s (SHS) core function is the supervision of the Housing Supervisor, and Housing Case Managers (HCM), as assigned, ensuring all housing program services and participant file documentation follows all contractual and regulatory obligations. 

 

SPECIFIC DUTIES:

1.       Provide regular supervision of the SHS and assigned HCM staff, through supervisory meetings and file monitoring and review, in accordance with funding regulations and agency policy and procedure. 

2.       Participate in the hiring process in accordance with agency policies that support program staffing.

3.       Mentor new staff to all program operations and compliance regulations, agency and program policy and procedures, and program goals and objectives, following the completion of agency trainer requirements.

4.       Maintain open and transparent communication with the Director of Residential Services.

5.       Ensure that the development and implementation of service plans and service plan reviews attached to each respective program are consistent with program goals and objectives and are of maximum benefit to the residents.

6.       Responsible for the creation of Admission Notes for residents entering Congregate Care Level II housing in accordance with program regulations and agency policy and procedures.

7.       Review participant files within 28 days of admission as a support to HCM’S and to ensure the file follows program regulations, policies, and procedures.

8.       Monitor, evaluate, and complete performance evaluations for SHS and HCM staff in accordance with agency policy and procedure.

9.       Identify problems related to resources, training, and personnel with a plan of corrective action.

10.   Responsible for the oversight of internal and external documents (i.e. assessments, ISP, SPR, and Physician’s Authorization’s renewals, MCO approvals, renewals, and appeals) and ensure accuracy and compliance, accepting those documents through signature.

11.   Provide written recommendations to policy development and/or revision based on experience and a working knowledge of agency policy, procedure or business practices.

12.   Keep abreast of regulatory, grant, and contractual changes.

13.   Meet with Quality Management for scheduled Utilization Management Reviews and Incident Review and participate in accordance with agency policy, procedure, or business practices.

14.   Develop and/or improve program components consistent with current best practice models and evidence-based solutions as part of a continuous quality improvement process.

15.   Ensure that programs operate in compliance with corporate standards and policy as well as contractual, local, state and federal program requirements. Coordinate with quality management to ensure preparedness for audits, submission of corrective action plans and resolution of findings.

16.   Cooperate fully with the Corporate Compliance Officer.

17.   Oversee the admission process to ensure that admission criteria are met, client funding sources are established or initiated prior to admission and the agency is represented at the weekly Elmira Psychiatric Center, Behavioral Services Unit and SPOE committees.

18.   Ensure that the program sites are maintained in a safe and clean manner and corrective action plans are developed to resolve property inspection findings.

19.   Monitor file documentation to ensure compliance with agency policy, procedure, or business practices, verifying file audits since last review, at case transfer and at discharge.

20.   Responsible for the day-to-day monitoring of the licensed programs and will participate in the QM&C scheduled monitoring upon request.

21.   Responsible for the initial household and/or room set-up and on-going monitoring of supplies for all Congregate Care Level II programs.

22.   Assists in the completion and submission of documents required for licensure and relocation.

23.   Chair weekly staff meetings and provide detailed agendas inclusive of agency announcements, community-based education / resources, and agency policy or procedure as applicable.

24.   Share back up, off hour support with other Program Supervisor(s), as assigned.

25.   Other duties as assigned.

 

QUALIFICATIONS:

Bachelor’s degree in human services or related field preferred

Five years’ experience in Human Services field and three years’ experience in a related supervisory position.

A combination of education and experience may be substituted at this discretion of the Executive Director.
