Location: 2462 Route 54A Suite 130, Penn Yan, NY
Employment Type: Full Time 35 Hours A Week

Salary: $19.00 an hour

About the Role:

The Child and Family Support Worker will fulfill the home visiting and skill building function within the SHAPE program. 

Key Responsibilities:

  • Carries a caseload that requires weekly visitation and crisis intervention in the client’s home environment. This may include schools, homes, clinics and other related sites. 
  • Ensures the rules and recommendations of the Office of Children and Family Services, Office of Mental Health, and the Agency as they are applicable to direct services and ASFA guidelines for children and families, are adequately and appropriately adhered to. 
  • Completes of all Family Development Plans or Individual Service Plans, and other supporting documents within the accepted timeframe outlined by standards. 
  • Relates to community agencies, coordinating services as needed to facilitate family rehabilitation, reunification and child permanency. 
  • Ensures compliance with each family’s service plan as outlined in the NYS Regulations and referral information through contacts with family members. 
  • Provides regular outreach family support and education sessions and suggests appropriate resources for family members. 
  • Maintains electronic reporting and program statistical information as required by the Agency and all funding sources. 
  • When necessary, supervises or observes visitation between family members to assure the safety and best interest of all members. 
  • Participate in in-service training to develop family casework skills. 
  • Supports, guides, mentors, coaches, &/or trains the child and/or family/caregiver in successful functioning in the home and community. 
  • Works one on one or in small groups (no more than 3 children) & their support networks. 
  • Works flexible schedules, as skill building activities may take place at any time 
  • of the day if the activities do not interfere with child's educational programs. 
  • Assists the child with the task completion, communication, academics, socialization, problem- solving, interpersonal skills, &/or decreasing negative behaviors. 
  • Proactively maintains current knowledge of the youth’s emotional and behavioral status, assesses the effectiveness of service plans, and completes timely documentation. 
  • Completes other duties as assigned 

 

Qualifications:

Education: Associates degree required. A high school diploma with commensurate experience may be considered. 

Credentials: Valid NY State Driver License and access to a reliable vehicle  

Experience: Prior human services experience, either through employment or educational internships required.  

·       Experience working with at risk youth, mental health, or child welfare highly preferred

  • Relevant combination of education and experience will be considered

·       Demonstration of the ability to unconditionally respect the abilities and diversity of all people while assisting them in developing and achieving personal goals related to the care of their children and personal development to maintain family unification. 

·       As a representative of the Agency, the FSW reinforces the implantation of the Agency’s philosophy and program. Their relationship with children and families and the community reflects the highest professional standards possible. 

·       Ability to prioritize assignments, plan, and complete work projects with minimal direction 

·       Exceptional time management skills and ability to effectively meet deadlines  

·       Excellent verbal/written skills 

·       Possession of a valid NYS Driver’s license and a driver's record considered acceptable by agency and insurance carrier. 

·       Continuous use of a reliable, registered and insured vehicle. 

·       Ability to work in the following software programs: Windows, Microsoft Office, and multiple online software platforms as required by funders and partner organizations 

·       Ability to maintain confidentiality and handle confidential information with discretion 

·       Ability to demonstrate behaviors and attitudes which support organizational mission, philosophy, and policies 

·       Ability to work in a cooperative and helpful manner with all individuals 

·       Willingness and ability to foster agency and program wide cooperation and teamwork through use of positive/constructive communication techniques
