Primary
Functions:
The Residential Supervisor’s core function is the supervision
of all staff within the Luce St Community Residence, overseeing the ongoing
care and support of the residents and ensuring all residential services and
operations are done in compliance with all contractual and regulatory
obligations.
Specific Duties:
·
Provide regular
supervision of the Assistant Residential Supervisor (ARS) and the Residential
Case Managers (RCM) through supervisory meetings and file monitoring and
review, in accordance with funding regulations and agency policy and procedure.
·
Participate in the
hiring process in accordance with agency policies that support program
staffing.
·
Mentor new staff to
all program operations and compliance regulations, agency and program policy
and procedures, and program goals and objectives, following the completion of
agency trainer requirements.
·
Ensure that the
development and implementation of service plans and service plan reviews
attached to each respective program are consistent with program goals and
objectives and are of maximum benefit to the residents.
·
Responsible for the
creation of Admission Notes for residents entering Congregate Care Level II
housing in accordance with program regulations and agency policy and
procedures.
·
Review resident files
within 28 days of admission as a support to RCM’s and to ensure the file
follows program regulations, policies, and procedures.
·
Monitor, evaluate, and
complete performance evaluations for RCM’s, in accordance with agency policy
and procedure.
·
Identify problems
related to resources, training, and personnel with a plan of corrective action.
·
Responsible for the
oversight of internal and external documents (i.e., assessments, ISP, SPR, and
Physician’s Authorization’s renewals) and ensure accuracy and compliance,
accepting those documents through signature.
·
Written
recommendations for policy development and/or revision based on experience and
a working knowledge of agency policy, procedure, or business practices.
·
Will meet with Quality
Management for scheduled Utilization Management Reviews and Incident Review and
participate in accordance with agency policy, procedure, or business practices.
·
Will monitor Resident
Case Manager file documentation to ensure compliance with agency policy,
procedure, or business practices. RCM will verify and file audits since last
SPR, at case transfer and at discharge.
·
Responsible for the
day-to-day monitoring of the licensed program and will participate in the
QM&C scheduled monitoring upon request.
·
Responsible for the
initial household and/or room set-up and on-going monitoring of supplies for
all Congregate Care Level II programs.
·
Assists in the
completion and submission of documents required for licensure and relocation.
·
Ensures the RCM’s
follow Administrative, Quality Management and Compliance policies, procedures,
or business practices.
·
Chair weekly RCM staff
meetings and provide detailed agendas inclusive of agency announcements,
community-based education / resources, and agency policy or procedure as
applicable.
·
Implement case reviews
on difficult cases needing collaboration for determination of admittance to
agency programs when appropriate.
·
Audit participant
files at Admission/discharge and ensure that data is accurate, and that
information is in the CAIRS database in a timely manner. Update related
worksheets (Telephone List, Rent Receipts, and Occupancy by Apartments) and
AWARDS residential data. Review, recode and forward participant outcome data
(LS3-6) in licensed programs to the Finance Department.

Provide timely
notification to the Department of Social Services and Social Security
Administration at time of discharge for all relevant participants.
·
Coordinate Luce St
admissions ensuring funding is in place/initiated, coordinating with ARS on
case management assignments, date/time of admission, apartment location, any
information related to coordinating the admission for all programs.
·
Continuously monitor
openings within Luce St to ensure that all programs are filled.
·
Review monthly
Occupancy Report for accuracy and report discrepancies to HMIS Administrator.
·
Provide back-up, when
needed, as agency representative to the weekly Elmira Psychiatric Center
review.
·
Provide back-up, when
needed, as facilitator at the Referral Meeting.
·
Assumes other duties
as assigned by supervisor.
ADDITIONAL REQUIREMENTS:
·
Possess excellent verbal
and written communication skills.
·
Ability to multi-task
and prioritize duties.
·
Willingness to foster
agency, department and program wide cooperation and teamwork through use of
positive/constructive communication techniques.
·
Ability to maintain
absolute confidentiality regarding all records reviewed including consumer
records, employee records and billing records.
·
Proficiency and
experience with PC’s and Microsoft applications.
·
Ability to analyze and
interpret data and to handle problem resolution.
·
Possession of a valid
NYS Driver’s license and a driver's record considered acceptable by agency and
insurance carrier.
·
Continuous use of a
reliable, registered, and insured vehicle.
·
Demonstrate commitment
to Agency Mission Statement.
·
Working knowledge of
community resources and funding systems external to the agency.
·
Background checks may be
required per program regulations.
·
Compliance: Adheres to
all applicable federal and state laws and regulations including, but not
limited to, those governing confidentiality, privacy, program, billing, and
documentation standards. All duties must be performed in accordance with CCDR’s
corporate compliance and ethics program.
Ability to meet the

Salary: $25.00-27.00/hour depending on
education and experience.
Schedule: Full-time

QUALIFICATIONS:
·
Associate degree in human services or
related field.
·
Three years’ experience in the Human
Services field and two years’ experience in a supervisor position.
·
A combination of education and experience
may be substituted at the discretion of the Executive Director.
