Position Title: HCBS
Peer Navigator
Salary: $18.00-19.00/hour
depending on years of experience/education level.
Schedule: Full time, Monday through Friday 35 hours
PRIMARY FUNCTIONS:
Sharing his/her own personal experience of recovery and using
a trauma-informed, non-clinical approach. The HCBS Peer Navigator will engage
in activities to assist individuals on the HCBS Waiver in initiating recovery,
maintaining recovery, and enhancing the quality of personal and family life in
long-term recovery.
SPECIFIC DUTIES:
Advocacy:
·
Assist individuals in seeking and obtaining benefits and
entitlements, food, shelter, permanent housing, etc.
·
Assist individuals with linkage to and system navigation
within behavioral health and allied human services systems to access
appropriate care.
·
Assist individuals in advocating for self-directed services.
Outreach
and Engagement:
·
Assist individuals in companionship and modeling of recovery
lifestyle, including participation in recovery activities that might be beyond
the scope of treatment providers (e.g., recovery celebration event, attending a
social event, etc.)
·
Raise the awareness of existing services, pathways to
recovery and helping individuals in removing barriers that may be blocking
access to his/her recovery.
Self-help
tools:
·
Assist individuals in selecting and utilizing self-directed
recovery tools such as Relapse Prevention
·
Planning and things that bring a sense of purpose and meaning
into his/her life.
·
Assist individuals in connecting to natural supports that
enhance the quality and security of life.
·
Assist individuals in making connections to self-help groups
in the community.
Recovery
Supports:
·
Provide recovery education and coaching for individuals on
the waiver and their family members.
·
Offer one-to-one peer support.
·
Assist the individual in assimilating into his/her community.
Transitional
Supports:
·
Provide a bridge for an individual leaving jail, prison, institution,
or general hospital and moving into a residential setting.
·
Assist the individual in assimilating into his/her community.
Pre-crisis
and Crisis Support:
·
Provide companionship when an individual is in an emergency
room or crisis unit or preparing to be admitted to detox, residential or other
services to deal with crisis.
·
Provide peer support in the individual’s home or in the
community to support them before (or in) a crisis or relapse.
·
Develop crisis diversion or relapse prevention plans.
General:
·
Maintain accurate documentation of all services provided to
individuals in accordance with standards set by funding sources in the
Electronic Health Record database.
·
Participate in team meetings, case conferences, and case
reviews.
·
Participate in staff, department, and agency meetings or
activities.
·
Represent Catholic Charities of Livingston County on
appropriate boards and committees as assigned by supervisor.
·
Assure compliance with all applicable local, State and
Federal regulations and agency policies.
·
Attend mandatory training.
·
Other duties as assigned by supervisor.
QUALIFICATIONS:
·
Peer Specialist Certification or Recovery Peer Advocate
Certification is required.
·
High School Diploma or GED with at least one year of relevant
experience working with substance use disorders and/or serious illness.
·
Must have knowledge of behavioral health systems and
resources.
·
Must have familiarity and sensitivity to mental health and
substance abuse issues.
ADDITIONAL
REQUIREMENTS:
·
Possess excellent verbal and written communication skills.
·
Previous experience working with diverse populations,
including low to moderate-income families.
·
Ability to multi-task and prioritize duties.
·
Willingness to foster agency, department and program wide
cooperation and teamwork through use of positive/constructive communication
techniques.
·
Ability to read, write, and interpret written documents.
·
Ability to maintain absolute confidentiality regarding all
records reviewed including consumer records, employee records and billing
records.
·
Proficiency and experience with PC’s and Microsoft
applications.
·
Ability to analyze and interpret data and to handle problem
resolution.
·
Possess a valid NYS Driver’s license and a driver’s record
considered acceptable by agency and insurance carrier.
·
Continuous use of a reliable, registered, and insured
vehicle.
·
Ability to travel in all weather conditions throughout the
counties served.
·
Bilingual (Spanish/English) desirable.
·

·

and regulations including, but not limited to, those governing confidentiality,
privacy, program, billing, and documentation standards. All duties must be
performed in accordance with CCDR’s corporate compliance and ethics program.

needs.
Ability
to meet the following physical requirements with or without reasonable
