PRIMARY FUNCTIONS: The Administrative Assistant will work with staff in the Care 

Management Program and will perform multiple office and computer related duties.


SPECIFIC DUTIES:

1. Maintain exceptional communication with all members of the care management team, as well as 

all other CCCS staff members, providers, volunteers, family members,

2. Copy, scan and upload documents as needed into the Electronic Health Record.

3. File documents in binders for care managers as needed.

4. Draft and send introduction letters to all new referrals.

5. Ensure all new referrals are tracked and distributed to care managers in a timely manner 

as directed.

6. Ensure that referral tracking/hospital database is kept up-to-date.

7. Ensure Gaps in Care tracking is up to date and notify care manager of timeframes.

8. Ensure all policy and procedure(s) for each Health Home as well as CCCS are updated and the 

agreed upon locations.

9. Assist with the development and keep updated, a Care Management Orientation book.

10. Ensure all contracts and grants are organized and up to date.

11. Ensure that caseload listings are kept current and distribute to identified staff.

12. Data entry as needed by Supervisor. If necessary, utilize each electronic record keeping system

13. Generate purchase orders as needed and follow procedure, as requested.

14. Ensure that waiting area is clean and presentable, containing relevant information.

15. Welcome all guests to the office suite.

16. Provide coverage, as needed to the agency’s main reception area.

17. Provide support to the care managers by confirming appointments with members, oversight of mailings 

to the member and their care team and tracking completions of necessary paperwork.

18. Perform all duties involved with scheduling staff trainings (registrations, accommodations, etc.).

19. Maintain correspondence with Interdisciplinary Team Members minimally on a monthly basis.

20. Run various weekly and monthly reports to ensure compliance with applicable regulations.

21. Other duties as assigned by supervisor


TITLE OF DIRECT SUPERVISOR: Care Management Supervisor.


QUALIFICATIONS:

EDUCATION: High School Diploma

EXPERIENCE: 2 years secretarial experience


SPECIAL REQUIREMENTS

• Experience with word, excel, power point, desired. Must be able to work with a team. Must be able 

to work with all types of people with a nonjudgmental lens

• The desired candidate will be the first person to greet incoming clients and other professionals, and

therefore must be friendly and welcoming. The ability to multi-task and transition from one task to 

another smoothly is essential.


ADDITIONAL REQUIREMENTS:

• Demonstrate ethical business practices, in conformance with all state and federal laws and regulations.

• Possess excellent verbal and written communication skills.

• Ability to multi-task and prioritize duties.

• Willingness to foster agency, department and program wide cooperation and team work through use of 

positive/constructive communication techniques.

• Ability to maintain absolute confidentiality in regard to all records reviewed including consumer records, 

employee records and billing records.

• Proficiency and experience with PC’s and Microsoft applications.

• Ability to analyze and interpret data and to handle problem resolution.

• Ability to work a flexible schedule as needed.

• Possession of a valid NYS Driver’s license and a driver's record considered acceptable by agency and 

insurance carrier.

• Continuous use of a reliable, registered and insured vehicle.

• Demonstrate commitment to Agency Mission Statement.

• Demonstrated knowledge of program compliance, regulations, and requirements of funding sources, 

HIPAA, and agency policies and procedures; including the Catholic Charities Confidentiality Statement.

• Working knowledge of community resources and funding systems external to the agency.

• Background checks may be required per program regulations.

• Compliance: Adheres to all applicable federal and state laws and regulations including, but not limited to, those 

governing confidentiality, privacy, program, billing and documentation standards. All duties must be performed 

in accordance with CCDR’s corporate compliance and ethics program.

Ability to meet the following physical requirements with or without reasonable accommodation:

• Use hands to manipulate, handle, feel, and control items or equipment, including motor vehicle;

• Stand, sit, walk, bend, stoop, kneel, and reach;

• Climb up or down stairs;

• Able to reach above or below shoulders;

• Occasionally lift or move objects weighing up to 10lbs;

• Sitting at a desk or in a vehicle for long periods of time to perform certain job functions;

Position Title:  Care Management Administrative Assistant

Salary: $18.50-hour depending on education and experience.

Schedule: Full Time: Monday-Friday

Location: Hoffman St. Elmira NY
