Children's Health Home Care Manager
PRIMARY FUNCTIONS:
Provides care
management services to individuals living in Wayne and surrounding counties
through assessment, referral, casework and crisis intervention within the
Children’s Health Home Program.

SPECIFIC DUTIES:
1.
Provide
comprehensive care management to children and families including but not
limited to completing a comprehensive assessment using CANS-NY (Child and
Adolescent Needs and Strengths); completing and updating child’s plan of care
that is person centered; utilizing multi-disciplinary teams in client care.
2.
Provide
care coordination and health promotion to child and families by coordinating
with service providers to make sure client needs are addressed.
3.
Assist
clients with applications and paperwork with management for benefits,
financial, housing, or other related areas as needed.
4.
Provide
referral to community and social support services by referring individuals to
community services and social supports according to their needs, tracking
referrals made, and collaborating and coordination with community-based
providers.
5.
Provide
comprehensive transitional care by transitional planning from one Health Home
to another, movement from one county to the next, appropriate linkage or
referral to care management prior to discharge, and follow-up on admissions to
a higher-level care setting such as a hospital.
6.
Proper
use of health information technology through care management agency adopting
HIT systems and practices as implemented by designated health homes, Medicaid
analytics performance portal (MAPP) and Regional Health Information
Organizations (RHIO).
7.
Serve
as client advocate.
8.
Provide
a core service for each client monthly as required or when needed, to assure
all care plan goals are addressed.
9.
Maintain
telephone availability for clients during the workday.
10.
Provide
crisis intervention and follow up.
11.
Maintain
current knowledge of professional practice through reading and reasonable
attendance at seminars, workshops and conferences.
12.
Complete
appropriate documentation as needed (CANS-NY assessments, Plan of Care
documents, Comprehensive Assessments, etc.).
13.
Provide
on-call availability via cell phone to the Health Home program as needed.
14.
Ensure
compliance with all applicable local, State, Federal and Heath Home Network
regulations and agency policies.
15.
Attend
agency functions as required.
16.
Participate
in staff meetings and meet monthly with supervisor for case review.

QUALIFICATIONS:
Bachelor’s
degree in Human Services or related field and two years of paid experience
working with persons with disabilities and/or chronic illnesses.
