Primary
Functions:
Under the supervision of the
Director of Emergency Services, the Housing Solutions Specialist (HSS) core
function is to develop a financial plan to assist those at-risk, in retaining
their present housing and any arrear payments for eviction prevention. The HSS
will also assist those who are homeless in rapidly regaining housing. The HSS
will identify and/or secure housing subsidies going forward in accordance with
the funding sources regulations. All participants being served will receive
case management provisions such as, but not limited to, direct services to
consumers that are at risk of becoming homeless. The HSS will conduct
assessments; develop and implement individual service plans; assist consumers
in achieving goals; teach and/or assist consumers on establishing stable
housing, communication, and self-advocacy skills; assist in accessing and
maintaining entitlements/benefits; assist residents in meeting the obligations
of tenancy.
Specific Duties:

The
HSS will meet regularly with the Emergency Services program coordinator (PC) to
review file documentation.
·
The
HSS is responsible for filing documentation in accordance with funding
regulations, agency policy, procedure, or business practice.
·
The
HSS is responsible for screening individuals and families requesting assistance
with homeless prevention services, the HSS will complete the intake/admissions
process will determine eligibility and communicate eligibility decision in a
dignified, timely fashion.

The
HSS will participate in agency orientation of program operations, policy,
procedure, goals, and objectives.
·
The
HSS will facilitate the integration of the resident into the community, using
community-based services whenever possible.
·
The
HSS will develop a financial plan of housing assistance in accordance with
funding sources regulations. Meet at minimum every 30 days or as needed with
participants receiving financial assistance through the rental assistant
funds.
·
The
HSS will complete accurate Housing Quality Standards Inspections (HQS) on each
location in receipt of the rental assistant funds. The HSS will identify the
best housing available at a rate equal to or less than the published FMR.
·
The
HSS maintains accurate case file records and reports based on the requirements
of each funding source/program.
·
HSS’s
will complete 6-month housing stability follow up via email, phone, face to
face, mailings, and document in the HMIS software system to report accurately
for quarterly reporting.
·
The
HSS will maintain accurate and pertinent records in Housing Information
Management System (HMIS).
·
The
HSS will act as a resource to ensure that the resident has access to services
and to ensure that the rights and confidentiality of the resident are not
overlooked or denied.
·
The
HSS is required to attend staff meetings, treatment team / provider meetings,
and staff development/training seminars in accordance with agency policy,
procedure, or business practice.
·
The
HSS is responsible for screening individuals and families requesting assistance
with homeless prevention services, the HSS will complete the intake/admissions
process will determine eligibility and communicate eligibility decision in a
dignified, timely fashion.
·
The
HSS will complete an initial assessment of the need to ensure immediate
prevention needs are taken care of to prevent homelessness and provide Case
Management to clients to increase housing stability.
·
Other
duties as assigned.
Position Title: Housing
Solutions Specialist
Salary: $20.00-20.50/hr. depending on
education and experience.
Schedule: Full Time, 35
hours/week.

QUALIFICATIONS:
·
Associate degree in
human services or equivalent experience. Two years of experience in Human
Services or related field.
·
A combination of
education and experience may be substituted at the discretion of the Executive
Director.
