Transportation and HOPE Youth Mentoring Caseworker
General Description
Coordination and implementation of programs including HOPE Youth Mentoring and Faith in Action along with providing support for department special events.
Location: 34 E. State street, Mt. Morris
Salary: $18.00-$19.00 an hour
Hours: Full Time 40 Hours a week
Essential Duties and Responsibilities
· Completes recruitment, screening, training and supervision of adult mentors.
· Supports mentors with “in-service” trainings, validation of mentor’s efforts, and physical and emotional support for mentors.
· Supervises and supports existing matches.
· Assesses the needs of referred individuals for both HOPE Youth Mentoring Program (HOPE) and the Faith in Action (FIA) program.
· Provides initial enrollment interview and intakes for those eligible for HOPE and FIA program services. Refers to other community resources to meet needs.
· Provides yearly assessment reviews for enrolled individuals in both HOPE and FIA programs.
· Delivers public presentations about HOPE to groups for recruitment purposes.
· Serves as liaison between referring agencies, schools families and mentors.
· Maintains files and documentation on current matches and unmatched youth, including Program Evaluations.
· Maintains accurate documentation and case management documentation in program database(s) for all program activities and participants.
· Completes and submits all monthly and annual program reports, stats, billing vouchers to grant contract funders per contract/grant specifics.
· Coordinates monthly program group outing/activities for matched and unmatched program youth.
· Provides transportation to/from group outings/activities for youth, as needed.
· Collaborates with Volunteer Coordinator with planning and implementing a yearly recognition event for mentors.
· Maintains open lines of communication with mentors and youth families.
· Attends agency annual functions as requested.
· Participates in staff meetings.
·Attends mandatory training.
· Other duties as assigned.
Qualifications
Education/Experience: Associates degree with at least two years’ experience working with youth and families preferred. High school diploma considered with qualified experience.

· Possession of a valid NYS Driver’s license and use of a registered and reliable vehicle
