Treatment Court Outreach Liaison
PRIMARY FUNCTIONS:
SPECIFIC DUTIES:
Community
Coordination
1.
Maintain oversight
and partner relationships with contract service providers, community resources
and service providers.
2.
Identify
innovative means of outreach to medical, education & other key partners
3.
Provide
outreach in the community to address trends, community needs and to maintain up
to date information regarding successes, participation, recidivism and general
community knowledge/awareness information.
4.
Maintain
databases and tracking systems that support identification of and response to
usage trends in Cayuga County.
5.
Serve as
point of contact between the Drug Treatment Court, community resources,
partners and general public.
6.
Maintain
statistical information and complete reporting as necessary.
Direct
Services
7.
Review individual’s cases, assess situation
and make appropriate referrals to contract agencies, community resources and
inter agency departments as needed.
8.
Establish (with client) and monitor
client action plans, conducting case reviews and team meetings as necessary.
9.
Conduct follow up with clients to
assure compliance with eligibility requirements
10.
Coordinate with Court Treatment
Team regarding status eligibility and ongoing requirements.
11.
Participate in Drug Treatment Court
Team Meetings, training, administrative and other activities as deemed
necessary by Court administration
Supports
Mission of the Agency
12.
Participate in staff meetings and
other CCFL-related meetings/events.
13.
Participate in Continuous Quality
Improvement efforts for the program and for the agency.
14.
Complete required agency and
program reports in a timely manner.
15.
Complete
other duties as assigned necessary to advance the response to substance abuse,
in particular heroin/opiate use and community needs in Cayuga County.
TITLE OF DIRECT SUPERVISOR: Director of
Services
QUALIFICATIONS:
EDUCATION: Bachelor’s
Degree in Social Sciences or Associates Degree with 2 years of related
experience, CASAC and advance degree preferred
EXPERIENCE: - Three to Five years’ experience in human
services
