CORE HCBS Adult Case Worker
PRIMARY FUNCTIONS:
CORE/HCBS Adult Case Workers provide person
centered, recovery oriented, mobile behavioral health supports intended to
build skills and self-efficacy that promote and facilitate community
participation and independence. Services are provided to Adults on Medicaid who
are enrolled in a HARP plan. Services
may include: Psychosocial Rehabilitation, Family Support and Training, Habilitation,
Pre-Vocational training, and Supported Employment. Functions may
include: intake, initial assessments, treatment planning, educating
individuals regarding available services and supports within the
broader CORE/HCBS
system, linkages to community
resources, support in obtaining various
benefits, documentation, service provision, and advocacy on behalf of
clients. All services are
provided in Client homes or in the community.
SPECIFIC DUTIES:
1.
Case Worker will
provide a variety of individualized CORE/HCBS services on a weekly basis to
eligible individuals in their homes or communities as assigned by the program
Director.
2.
Review referral
information for CORE/HCBS services as provided by program Director, MCO’s,
Health Home Care Managers, and any other referral sources.
3.
Complete the agency intake and assessment process and determine
potential goals together with the client for development of a Person Centered,
Strengths Based, Individualized Service Plan.
4.
Explain CORE/HCBS
Services and coordinate collection of necessary
ID and required documents, if offer
of services is accepted.
5.
Responsible for the submission of authorization requests
with MCO’s for initiation and continued service provision in a timely manner.
6.
Will document all
services provided within the client file in the agency Electronic Health Record
in a timely fashion so that billing for services provided may occur.
7.
May work together
with other agency staff to provide group experiences to interested clients.
8.
Coordinate with staff
from other agencies on a regular basis to help
ensure coordination of care for clients.
9.
Perform duties associated with
agency policy of
reporting abuse and neglect as established by Department
process and standards.
10.
Will meet weekly
with program director for supervision.
11.
Participate in
agency team meetings.
12.
Attend agency
functions, as requested.
13.
Attend all mandatory
training within assigned deadlines.
14.
Other duties as
assigned.
TITLE OF DIRECT SUPERVISOR: Director of Adult Behavioral Health Services
PREFERRED QUALIFICATIONS:

|
1.
Bachelor degree in Human Services or
related field or 5 years’
experience
working with
persons with Behavioral Health diagnoses OR
Substance Abuse challenges
2.
Experience with
SSI/SSDI and/or Medicaid
benefits preferred
|

REQUIRED QUALIFICATIONS:
1. 18 years of age and have a high school diploma
or equivalent
2. 1-3 years of experience in a behavioral health
or substance abuse setting
3.
Knowledge of or
willingness to become
familiar with CORE/HCBS services
4.
Direct service staff must possess appropriate
skills for engaging clients and family members; providing education about substance
use disorder/mental illness and its treatment; possess information on community
resources; and be skilled in the provision of guidance on how to manage or cope
with substance use disorder relapse, maladaptive behaviors; emotional support
and counseling; crisis planning; and problem-solving skills training.
5.
Staff must be willing to provide Trauma informed
services from a person centered, strengths based approach and skilled at
working within a team.

Ability to maintain absolute
confidentiality of all records reviewed including consumer records, employee
records and billing records.

Proficiency and experience with
PC’s and Microsoft applications.
·
Ability to analyze and
interpret data and to handle problem resolution.
·
Possession a valid NYS Driver’s
license and a driver's record considered acceptable by agency and insurance
carrier.
·
Continuous use of a reliable,
registered and insured vehicle.
·
Demonstrate full adherence to
the CCSL Code of Conduct and all policies/procedures related to compliance.
·
Read write and interpret
written documents;
·
Demonstrate commitment to
