Treatment Court Intensive Case Manager
Primary
Functions: The
Intensive Case Manager is
responsible for identifying, screening, motivating, and monitoring clients as
well as linking them with appropriate mentors and services and sharing weekly
information updates with the drug court team.
SPECIFIC DUTIES:
1.
Provide intensive case management to 15 at-risk participants
per year. Work directly with
participants, who will be required to check-in frequently, and closely monitor
participants for safety and support.
2.
Meet with participants in various settings such as
participant’s home or community locations.
3.
Provide crisis-specific interventions, address coping
skills, coordinate access to health services and assist with housing, social,
vocation, education, and other wrap-around service needs.
4.
Provide one-on-one assistance to help with daily living
skills and assure linkage to other community resources.
5.
Maintain ongoing communication with the Treatment Court
Coordinator of participants’ progress.
6.
Work with project staff to develop and implement a simple
screening tool to identify overdose risk.
7.
Assist in collecting and maintaining all data as required by
BJA.
8.
Ensure
participants attend to court appearances, off-site referrals, sober support meetings
and community services, as needed.
9.
Participate
in agency staff meetings and other agency-related meetings/events.
10. Participate
in agency’s strategic planning efforts.
11. Complete
required agency and program reports in a timely manner.
12. Perform
other related duties as required/requested.

Treatment
Court Coordinator (day to day)
QUALIFICATIONS:
Bachelor’s
degree in field of criminal justice, social work, counseling, psychology,
sociology, criminology.
Credentialed
Alcohol and Substance Abuse Counselor is strongly preferred.
Minimum of 3
years work experience in counseling, substance use treatment or case
management,
Or- have an
equivalent combination of education and experience.
ADDITIONAL
REQUIREMENTS:

Possess excellent verbal and written communication skills.
·
Ability to multi-task and prioritize duties.
·
Willingness to foster agency, department and program wide
cooperation and teamwork through use of positive/constructive communication
techniques.
·
Ability to maintain absolute confidentiality of all records
reviewed including consumer records, employee records and billing records.
·
Proficiency and experience with PC’s and Microsoft
applications.
·
Ability to analyze and interpret data and to handle problem
resolution.
·
Possession of a valid NYS Driver’s license and a driver's
record considered acceptable by agency and insurance carrier.
·
Continuous use of a reliable, registered and insured
vehicle.
·
Willingness to travel within the region.
·
Hours will require some evening and weekend hours.
