Lead Family Coordinator

Position: Lead Family Coordinator
Program: Family Development
Reports to: Family Development Director
Status: Full Time, Non-Exempt, 37.5 hours per week
Schedule: Monday through Friday
Hourly Wage: $25.00 - $27.00/hour

**Temporary grant funded position, until September 30, 2028**

*This position works in partnership with Cortland County Mental Health Department*

Job Function: A key personnel position responsible for leading family engagement, family support, and family-centered coordination activities under the SAMHSA Community Mental Health Infrastructure (CMHI) Grant. In collaboration with the Project Director, clinical and community partners, peer/family support staff, and other grant personnel, the Lead Family Coordinator develops and maintains systems that promote authentic family voice, strengthen family participation, improve access to behavioral health and community resources, and support positive outcomes for children, youth, adults, and families. This position will play a critical role in achieving grant objectives and maintaining fidelity to SAMHSA’s principles of family-driven, person-centered, trauma-informed, culturally responsive, and recovery-oriented care.


Essential Duties and Responsibilities

SAMHSA CMHI Grant Implementation

 

·         Collaborate and maintain regular communication with the Project Director and grant team to monitor progress and advance family-related objectives, identifying challenges and emerging needs.

 

·         Participate in required grant meetings, trainings, technical assistance activities, and stakeholder engagement efforts, including leadership and implementation meetings.

 

·         Maintain documentation and assist with reports, progress updates, success stories, and other materials needed to demonstrate grant compliance, performance measures, and project evaluation outcomes.

 

·         Support preparation for SAMHSA monitoring, audits, site visits, and other grant oversight activities as applicable

 

·         Contribute to sustainability planning and the integration of successful family engagement practices into ongoing program operations.

 

·         Develop and maintain strong relationships with family-serving organizations, peer organizations, schools, healthcare providers, behavioral health agencies, social service agencies, and other community partners.

 

·         Participate in community coalitions and collaborative initiatives that advance the goals of the CMHI project.

 

·         Represent the organization and CMHI project at community meetings, outreach events, conferences, and stakeholder activities.

 

·         Utilizing family feedback and program data, identify opportunities to expand family access to community resources and strengthen the local behavioral health continuum of care to improve accessibility, quality, and outcomes.

 

 

Family Support, Engagement & Coordination

 

·         Serve as the primary liaison between the CMHI initiative and participating families.

 

·         Lead assigned family engagement activities and ensure completion of related goals, milestones, and deliverables, monitoring family engagement activities against the approved project work plan and promoting peer-to-peer learning opportunities.

 

·         Develop and implement strategies that ensure meaningful family engagement in program planning, implementation, evaluation, and continuous quality improvement.

 

·         Identify barriers affecting families and collaborate with program leadership and community partners to develop solutions.

 

·         Provide or coordinate family support, education, resource navigation, and referrals consistent with CMHI objectives.

 

·         Assist families in accessing behavioral health, educational, healthcare, nutrition, housing, social service, and other community resources, promoting family leadership and participation in decision making.

 

·         Provide technical assistance related to family engagement, family-driven care, trauma-informed practices, behavioral health resources, and effective family-provider collaboration, supporting the development of culturally responsive and family-centered approaches.

 

·         Foster coordination among families, behavioral health providers, schools, healthcare organizations, social service agencies, and community partners to reduce service gaps and duplication.

 

·         Ensure family support services are responsive to each family's unique circumstances, strengths, preferences, and goals.

 

·         Promote equitable access to services for underserved and historically marginalized populations.

 

·         Establish and facilitate family advisory groups, focus groups, listening sessions, and other mechanisms to gather and elevate family feedback.

 

·         Ensure family perspectives inform policies, procedures, service delivery, outreach efforts, and program improvements.

 

·         Collect and maintain accurate information related to family engagement activities, participation, referrals, services, and outcomes, consistent with organizational policies and grant requirements.

 

·         Gather qualitative and quantitative feedback from families to assess program effectiveness and identify opportunities for improvement.

 

 

Education and/or Experience

High School Diploma/GED required.  Associate's degree (A. A.) preferred or equivalent from two-year College or technical school; plus, three years’ experience in social work, human services or related field with youth with serious emotional disorders (SED); or equivalent combination of education and experience.

 

Experience as a parent or caregiver of a child or adolescent with SED who has received or is currently receiving services from the mental health system preferred. Experience in developing and coordinating community initiatives to provide maximum participation through planned outreach activities.


Other Skills and Abilities

Experience in working with individuals, neighborhood groups (youth and adults) and community organizations. Ability to grasp the problems confronting low-income residents, and families experiencing trauma and SEDs. Working knowledge of the family development process.


Certificates, Licenses, Registrations

Valid NYS driver's license with acceptable driving record. Must be able to successfully complete a pre-employment background check.
