Position Title:  Children’s Health Home Care Manager

Salary: $20.00-22.00/hour depending on years of experience/education level.

Schedule: Full time, Monday through Friday


PRIMARY FUNCTIONS:

Provides care management services to children living in Livingston, Steuben, and Wyoming counties who have

been diagnosed with one or more chronic illnesses, a serious emotional disturbance, OR HIV/AIDS and

who meet appropriateness criteria.


SPECIFIC DUTIES:

1. Provide direct care management services maintaining accurate and verifiable documentation of all services rendered.

2. Conduct visits with clients in their chosen setting:  home, school, or community approximately three days per week.

3. Accept referrals as assigned by Program Manager in Livingston, Steuben, and/or Wyoming Counties.

4. Use assigned office as home base:  Mt. Morris, Hornell, or Bath location.

5. Complete comprehensive and detailed assessments, reviews, crisis plans, and care plans as required by standards set by DOH and lead Health Home contractors.

6. Will conduct additional assessments including, but not limited to, Child and Adolescent Needs and Strengths –New York (CANS-NY), Safety/Crisis, and Comprehensive Assessments.

7. Oversee implementation of plans to the satisfaction of the clients served.

8. Provide one to two core services for each client monthly as required or when needed, to assure all care plan goals are addressed.

9. Maintain telephone availability for clients during the workday.

10. Assist clients with applications and paperwork for and management of benefits, financial, housing, or other related areas as needed.

11. Arrange for and manage coordination of medical care and other services as needed for clients.

12. Assist clients in accessing community-based supports.

13. Communicate with management staff regarding intakes and placement of clients within each program.

14. Ensure adequate coordination, appropriate communication and maximum cooperation between all sources of support and services.

15. Provide on-call availability via cell phone to Health Home program as needed.

16. Serve as client advocate.

17. Facilitate/attend case conferences and case reviews.

18. Participate in team meetings.

19. Ensure compliance with all applicable local, State, Federal and Health Home Network regulations and agency policies.

20. Attend all trainings as required by DOH, Health Homes, and agency in a timely manner.

21. Represent Catholic Charities Steuben/Livingston on appropriate boards and committees as assigned.

22. Perform outreach duties in all counties served by the program, including, but not limited to: tabling at community events, educating partner agencies about care management services, reaching out to potential referral sources, attending community meetings and events which have been identified as

23. potential referral sources by supervisor.

24. Attend agency functions as required.

25. Other duties as assigned.


TITLE OF DIRECT SUPERVISOR: Children’s Health Home Care Management Supervisor


QUALIFICATIONS:

Bachelor’s degree in Human Services or related field and two years of paid experience working with persons with disabilities and/or chronic illnesses.

OR

Bachelor’s degree in an unrelated field with five years of paid experience working with persons with disabilities and/or chronic illnesses.

OR

Associate’s degree in Human Services or related field and two years of paid experience working with persons with disabilities and/or chronic illness.


ADDITIONAL REQUIREMENTS:

Demonstrate ethical business practices, in conformance with all state and federal laws and regulations.

Possess excellent verbal and written communication skills.

Previous experience working with diverse populations, including low to moderate-income families.

Ability to multi-task and prioritize duties.

Ability to travel in all weather conditions throughout the counties served.

Willingness to foster agency, department and program wide cooperation and teamwork through use of positive/constructive communication techniques.

Ability to maintain absolute confidentiality of all records reviewed including consumer records, employee records and billing records.

Proficiency and experience with PC’s and Microsoft applications.

Ability to analyze and interpret data and to handle problem resolution.

Possession a valid NYS Driver’s license and a driver's record considered acceptable by agency and insurance carrier.

Continuous use of a reliable, registered and insured vehicle.

Demonstrate full adherence to the CCLC Code of Conduct and all policies/procedures related to compliance.

Read write and interpret written documents;

Demonstrate commitment to Agency Mission Statement.

Ability to meet the following physical requirements with or without reasonable accommodation:
