Position Title: Jail
Services Reentry Case Worker
Salary: $18.00/hr.
Schedule: Full time, 35 hours per week
PRIMARY FUNCTIONS:
Provide transitional case management services to aid in the
reintegration of individuals being released from Livingston County Jail and
residing in Livingston or Steuben County.
Monitoring for up to 12 months post release with at least bi-weekly
meetings to assist with clients “Transitional from Jail to Community”
plans. Targeting referrals to employment
programs, housing assistance, mental health, parenting skills, case management,
client advocacy, collaboration with Livingston County Jail and Livingston
County Probation.
SPECIFIC DUTIES:
· Provide level of service
inventory assessments to inmates.
· Conduct individualized
and group orientation to include knowledge of community resources including but
not limited to benefit applications, housing opportunities and transportation
services to assist with successful reentry.
· Facilitate presentations
by community service providers on an as needed basis.
· Assist with SSI-SSDI
applications using the SOAR process.
· Determine eligibility
for Health Homes and HARP assessment and referral.
· Model, reinforce,
redirect, and set limits to behavior by using specified behavior management
techniques; develop and implement individual goal attainment.
· Schedule inmates for
specialized appointments in preparation of transition date.
· Report measurable
outcomes including but not limited to total number of clients screened, total
number of criminogenic needs assessments completed, units of services provided
and total number of types of service referred to (mental health, PCP, DSS,
Catholic Charities, CASA, as well as other community support group/service) to
the CCS/L Executive Director.
· Develop “Transition from
Jail to Community” (TJC)plans upon the inmate’s release to the community.
· Maintain all
documentation of screening, assessments, and individualized service plans in
CCS/L's secure client database.
· Participate in staff meetings.
· Attend agency functions,
as requested.
· Attend mandatory
training.
· Other duties as
assigned.
QUALIFICATIONS:
Associate
degree in social work, psychology, criminology, vocational rehabilitation,
education, guidance or in a behavioral science involving emphasis human
relations. A combination of education
and experience may be substituted at the discretion of the Executive Director.
ADDITIONAL
REQUIREMENTS:
- Possess excellent verbal and written communication
skills.
- Ability to multi-task and prioritize duties.
- Willingness to foster agency, department and program
wide cooperation and teamwork through use of positive/constructive
communication techniques.
- Ability to maintain absolute confidentiality regarding
all records reviewed including client records, employee records and
billing records.
- Proficiency and experience with PC’s and Microsoft
applications.
- Ability to analyze and interpret data and to handle
problem resolution.
- Possessing a valid NYS Driver’s license and a driver's
record considered acceptable by agency and insurance carrier.
- Continuous use of a reliable, registered, and insured
vehicle.
- Read, write and interpret written documents.
- Demonstrate commitment to Agency Mission Statement.
- Compliance: Adheres to all applicable federal and state
laws and regulations including, but not limited to, those governing
confidentiality, privacy, program, billing, and documentation standards.

Ability
to meet the following physical requirements with or without reasonable

- Able to reach above or below shoulders.
