to 18.00 an hour

About
the Role:
To
provide case management services to better assist the client with working
through the SSI eligibility process for a successful conclusion.
Key Responsibilities:
1. Establish and
maintain supportive relationship with participants as requested via referrals
from the local Dept. of Social Services (LDSS), primarily through office visits
and/or home visits, telephone visits and written correspondence.
2. Notify LDSS when
clients are enrolled in program, have failed to participate or have withdrawn
from program.
3. Contact LDSS upon
discovery of any factors that may negatively influence the participant’s
movement toward obtaining SSI benefits.
4. Interface with various health care
providers to clarify the purpose of the medical information they are being
asked to provide. This may include physicians, therapists, psychologists,
Veteran’s hospitals, hospitals, clinics, prison/jail systems, substance abuse
providers and school records.
5. Interface with Legal
Services and other providers to identify/coordinate/arrange for/advocate
for/provide any additional information for appeal of an SSI denial.
6. Build the SSI case by
accessing relevant records, including third party statements and LDSS case
notes.
7. Evaluate work history
and ability for substantial gainful employment by identifying opportunities for
retraining/alternate employment.
8. Complete and submit
the SSI application packet.
9. Arrange for or
provide transportation for hearings.
10. Assist, and
coordinate if necessary, the SSI applicant in obtaining quality medical
evaluations. This includes having an awareness of specialists and learning
disability screening when appropriate.
11. Actively engage the
SSI applicant into appropriate treatment.
12. Maintain case
records and statistics as required.
13. Prepare timely
reports for CCFL and LDSS.
14. Participate in staff
meetings and other agency-related meetings/events.
15. Participate in
agency’s strategic planning efforts.
16. Complete required
agency and program reports in a timely manner.
17. Perform other
related duties as required/requested.
Qualifications:
- Requires a Registered Nurse or an associate’s degree in
another medical profession. Bachelor’s Degree in related medical support
field preferred.
- EXPERIENCE:
1-3 years of experience in human services, especially case or care
management. Familiarity and understanding of SSI rules and eligibility
guidelines.
Relevant combination of
education and experience will be considered
Preferred Skills:
- Possess excellent verbal and written communication
skills.
- Ability to multi-task and prioritize duties.
- Willingness to foster agency, department and program
wide cooperation and teamwork through use of positive/constructive
communication techniques.
- Ability to maintain absolute confidentiality in regard
to all records reviewed including consumer records, employee records and
billing records.
- Proficiency and experience with PC’s and Microsoft
applications.
- Ability to analyze and interpret data and to handle
problem resolution.
- Possession of a valid NYS Driver’s license and a
driver's record considered acceptable by agency and insurance carrier.
- Continuous use of a reliable, registered and insured
vehicle.
- Willing to travel
and to attend evening meetings
- Demonstrate commitment to Agency Mission Statement.

laws and regulations including, but not limited to, those governing
confidentiality, privacy, program, billing and documentation standards.
