Position Title: Healthy
Families Program Supervisor
Salary: $18.00-22.00/hr
Schedule: Full time/35 hours per week
PRIMARY FUNCTIONS:
Program Supervisors are responsible for the direct
supervision of assigned workers. They actively participate in the development
and implementation of the program and work collaboratively with area service
providers to make sure the goals of the program are met.
SPECIFIC DUTIES:
· Provides structured as
well as informal direct supervision to all assigned staff.
· Assists staff in
supporting families to become empowered to identify strengths and set goals.
· Performs quality
assurance tasks such as reviewing records and surveying participants.
· Monitors and manages
staff caseloads.
· Assists in staff
selection and orientation as well as facilitating on-going training.
· Monitors staff
performance
· Ensures tracking forms
and IFSPs are completed for each family.
· Responsible for data
collection and general management at assigned site.
· Monitors staff
performance
· Functions as team leader
by conducting team meetings and planning groups.
· Provides daily support
and crisis management.
· Assists in the
development of policies and procedures.
· Works closely with all
referral sources and community agencies to ensure families’ needs are being
met.
· Reviews cases on a
regular basis and assures that any case which may involve “imminent harm” is
referred to CPS, as well as reviews and approves all CPS reports made by staff.
· Attending conferences
and training as required.
· Performs other related
duties as assigned.
QUALIFICATIONS:
Master’s degree in human service field preferred, or
bachelor’s degree with five years of experience with a home visiting program.
FSWs or FAWs with five years direct service in a Healthy
Families Program, a documented history of progressive professional development,
plus an associate degree in a related field may be considered for supervisory
positions.
ADDITIONAL REQUIREMENTS:
- Sense of acceptance and fairness, with the ability to
empower and nurture.
- Administrative experience in a human service program,
including experience in quality assurance and program improvement and site
development.
- A solid understanding of and experience in managing
staff.
- Knowledge of maternal-infant health and concepts of
child abuse and neglect.
- Solid understanding and experience in managing and
motivating staff as well as providing support in stressful working
environments.
- Experience in home visiting and working with multi-needs
families.
- Experience with and commitment to reflective practice.
Infant mental health endorsement level III or IV a plus.
- Knowledge and experience in a strength-based and family
centered provision of primary prevention services, and/or direct
experience as a home visitor and/or home visitor supervisor.
POSITION
DESCRIPTION
- Adheres to all applicable federal and state laws and regulations
including, but not limited to, those governing confidentiality, privacy,
program, billing, and documentation standards. All duties must be
performed in accordance with CCDR’s corporate compliance and ethics
program.
- Possess excellent verbal and written communication
skills.
- Ability to multi-task and prioritize duties.
- Willingness to foster agency, department and program
wide cooperation and teamwork through use of positive/constructive
communication techniques.
- Ability to maintain absolute confidentiality in regard
to all records reviewed including consumer records, employee records and
billing records.
- Proficiency and experience with PC’s and Microsoft
applications.
- Ability to analyze and interpret data and to handle
problem resolution.
- Possession of a valid NYS Driver’s license and a
driver's record considered acceptable by agency and insurance carrier.
- Continuous use of a reliable, registered, and insured
vehicle.
- Demonstrate commitment to Agency Mission Statement.

respectful quality care and services that are responsive to diverse
cultural health beliefs and practices, preferred languages, health
literacy and other communication needs.
Ability
to meet the following physical requirements with or without reasonable
accommodation:
- Use hands to manipulate, handle, feel, and control items
or equipment, including motor vehicles.
- Stand, sit, walk, bend, stoop, kneel, and reach.
- Climb up or down stairs.
- Able to reach above or below shoulders.
- Occasionally lift or move objects weighing up to 10 lbs.
