Jail Services Case Worker

PRIMARY FUNCTIONS:

Performs specialized social service duties at the Livingston County Jail; follows established prescribed policies and procedures in the rehabilitation of inmates; provides counseling either individual or in groups which may assist inmates in their transition into a community environment; does related work as required including case management and parent education, working in close and immediate contact with inmates daily, while maintaining security.

SPECIFIC DUTIES:

1. Provide level of service inventory assessments to inmates.

2. Conduct individualized and group orientation to include knowledge of community resources including but not limited to benefit applications, housing opportunities and transportation services to assist with successful reentry.

3. Facilitate presentations by community service providers on an as needed basis.

4. Assist with SSI-SSDI applications using the SOAR process.

5. Determine eligibility for Health Homes and HARP assessment and referral.

6. Coordinate medical/mental health appointments (as needed).

7. Participate as a member of the Jail Team that monitors inmates to ensure a safe and secure environment.

8. Attend jail staff meetings.

9. Model, reinforce, redirect and set limits to behavior by using specified behavior management techniques; develop and implement individual goal attainment.

10. Offer and provide parenting classes within the Livingston County Jail or in the community one time per week

11. Observes inmates and collaborate with other staff and community agencies to identify gaps and help develop services for offender reentry needs.

12. Schedule inmates for specialized appointments in preparation of transition date.

13. Report measurable outcomes including but not limited to total number of clients screened, total number of criminogenic needs assessments completed, units of services provided and total number of type of service referred to (mental health, PCP, DSS, Catholic Charities, CASA, as well as other community support group/service) to the CCLC Executive Director, County Jail Deputy and OMH Director as needed.

14. Coordinate with professional staff (psychiatrist, social worker and psychologist) to ensure treatment for those offenders needing specialized services upon transition.

15. Develop aftercare plans upon the inmate’s release to the community.

16. Track individual prior offenses (if any), and level of current offense to aid in determining risk of reoffending.

17. Maintain all documentation of screening, assessments, and individualized service plans in CCLC's secure client database.

18. Participate at staff meetings

19. Attend agency functions, as requested.

20. Attend mandatory training.

21. Offer and provide cognitive behavioral intervention classes to inmates one to two times per week

22. Other duties as assigned.

QUALIFICATIONS:

Bachelor’s degree in social work, psychology, criminology, vocational rehabilitation, education, guidance or in a behavioral science involving emphasis on counseling and human relations, Master’s level preferred. One year of previous counseling experience. A combination of education and experience may be substituted at the discretion of the Executive Director
