Children’s Health Home Care Management Supervisor
PRIMARY FUNCTIONS: Provides care management services to children living in Livingston and Steuben counties who have been diagnosed with one or more chronic illnesses, a serious emotional disturbance, OR HIV/AIDS and who meet appropriateness criteria
Location: 34 E. State St, Mt.Morris NY
Salary: Annually $60,000-65k
Schedule: Full Time Exempt
SPECIFIC DUTIES:
1. Provide direct care management services maintaining accurate and verifiable documentation of all services rendered. The Supervisor maintains a minimal caseload, in reasonable proportion to number of care managers being supervised.
2. Complete comprehensive and detailed assessments, reviews, crisis plans, and care plans as required by standards set by DOH and lead Health Home contractors.
3. Will conduct additional assessments including, but not limited to, Child and Adolescents Needs and Strengths –New York (CANS-NY), Safety/Crisis plans, and Comprehensive Assessments.
4. Direct hiring and training of all new care managers.
5. Provide direct supervisory support to care managers.
6. Manage and approve timesheets for all care managers.
7. Attend team meetings and/or home visits with care managers as needed.
8. Ensure assessments/monthly work for all children is completed in a timely manner, and work with individual care managers if they require assistance in completion or time management.
9. Maintain accurate records of internal and external audits, which can be completed by the Health Home, the Managed Care Organizations, and the Care Management Agency.
10. Communicate with Health Home quality management teams regarding closure of GAPS in care.
11. Report financial and billing information monthly to agency finance and Health Homes.
12. Hold care management team meetings.
13. Management and system input of all referrals from Health Homes and outside agencies and community partners.
14. Oversee implementation of care plans to the satisfaction of the clients served, review/sign all care plans and other documentation written by care management team.
15. Provide a core service (or ensure core service is provided by care manager) for each client monthly as required or when needed, to assure all care plan goals are addressed.
16. Determine when cases need to be closed by holding utilization review meetings when needed.
17. Maintain telephone availability for clients and care managers during the workday.
18. Provide on-call availability via cell phone to Health Home program as needed.
19. Serve as client and care manager advocate.
20. Facilitate/attend case conferences and case reviews.
21. Participate in agency team meetings and functions.
22. Ensure compliance with all applicable local, State, Federal and Health Home Network regulations and agency policies.
23. Attend trainings as required by DOH, Health Homes, and agency in a timely manner.
24. Attend supervisory trainings as required by DOH and the Health Homes.
25. Represent Catholic Charities Steuben/Livingston on appropriate boards and committees as assigned.
26. Other duties as assigned by supervisor.
TITLE OF DIRECT SUPERVISOR: Director of Daily Operations
QUALIFICATIONS:
Bachelor’s degree in Human Services or related field and two years of paid experience working with persons with disabilities and/or chronic illnesses.
OR
Bachelor’s degree in an unrelated field with five years of paid experience working with persons with disabilities and/or chronic illnesses.
In addition, Child and Adolescents Needs and Strengths (CANS-NY), Community Mental Health Assessment (CMHA) and Health and Recovery Plan (HARP) may be required.
Bilingual (English/Spanish) skills may be required for some positions.
ADDITIONAL REQUIREMENTS:
· Demonstrate ethical business practices, in conformance with all state and federal laws and regulations.
· Possess excellent verbal and written communication skills.
· Previous experience working with diverse populations, including low to moderate-income families.
· Ability to multi-task and prioritize duties.
· Ability to travel in all weather conditions throughout the counties served.
· Willingness to foster agency, department and program wide cooperation and teamwork through use of positive/constructive communication techniques.
· Ability to maintain absolute confidentiality of all records reviewed including consumer records, employee records and billing records.
· Proficiency and experience with PC’s and Microsoft applications.
· Ability to analyze and interpret data and to handle problem resolution.
· Possession a valid NYS Driver’s license and a driver's record considered acceptable by agency and insurance carrier.
· Continuous use of a reliable, registered and insured vehicle.
· Demonstrate full adherence to the CCLC Code of Conduct and all policies/procedures related to compliance.
· Read write and interpret written documents;

Ability to meet the following physical requirements with or without reasonable accommodation:
